"MEET & GREET" VISIT:
- All
prospective new adult
patients
are required to meet with
the doctor for a short introductory visit.
- The purpose of
this visit is :
- To
personally meet all prospective new patients.
- To
briefly
discuss the past medical history;
- To
briefly discuss the present health status;
- To
discuss the expectations and needs of the patient;
- To
discuss any of the clinic policies the patient may be unsure of;
- To
list current medications
being taken;
- To
list medical concerns
which needs to be
addressed during
future visits.
- Please
be aware that this first meeting is NOT a clinical visit.
No clinical examinations or
treatments or referrals to other health care providers will be done
for any condition during this visit.

An appointment for a
"meet and greet" visit does not imply that the prospective new
patient will be accepted into the doctor's practice.
- The
doctor will only accept patients into his / her practice provided he /
she is of the opinion that a sound doctor - patient relationship can be
established.
- Accepted patients will be asked to
make an appointment for a
general health check. (Please note that this appointment is for a
health check, and not to address specific problems.)
- Accepted patients
will need to make additional appointments to address specific
medical problems they may have.
- Please
arrive
15 minutes early to complete a registration form and a health
questionnaire before you see the doctor. You may also
download these forms directly from our website and complete them
ahead of time.
- Please bring along :
- Alberta Health
Card
- Picture ID.
- All
prescription medications you are using.
- If you
can not attend your appointment for any reason: PLEASE CANCEL AHEAD
OF TIME!!!. Individuals not showing up for their meeting, will not be accepted afterwards.
